Thank you for attending the 55th ASH Annual Meeting and Exposition in New Orleans, LA. We hope that you gained new and valuable scientific and educational information that you can apply to your medical practice.

The American Society of Hematology is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

The American Society of Hematology designates this live activity for a maximum of 37.5 AMA PRA Category 1 CreditsTM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

By completing this Online Evaluation, you document your attendance at the meeting, which is necessary to receive an official Continuing Medical Education (CME) Certificate. You will also provide ASH with valuable feedback to be used in planning future CME activities.

EHA-CME Credit
Any healthcare professional with a strong interest in hematology or related fields attending one or more of the EHA-CME accredited sessions may request EHA-CME Credit Points. The 55th ASH Annual Meeting program content has been reviewed and approved for accreditation by the European Hematology Association (EHA) CME Unit. There is no fee required in order to be eligible for EHA-CME Credit Points. For additional information on claiming EHA-CME Credit Points, visit the CME Unit of the EHA website, http://eha-cme.org/. EHA CME Credit Points for the 55th ASH Annual Meeting can be claimed through the EHA website beginning Friday, December 27, 2013.

Physicians
Only physicians are eligible to be awarded AMA PRA Category 1 CreditTM. The American Medical Association (AMA) defines physicians as those individuals who have obtained an MD, DO or equivalent medical degree from another country.
Physicians who participate in this CME activity but are not licensed in the U.S. are also eligible for AMA PRA Category 1 CreditsTM.

Non-Physicians
Non-physicians and other health-care professionals attending the meeting can receive a Certificate of Attendance as well as a summary report of CME-accredited sessions attended. There is no fee required for this service. Please click here to be redirected to the Certificate of Attendance site.

Fees
There is a $25 processing fee per physician attendee for a CME certificate. There is no fee required in order to be eligible for EHA-CME Credit Points or to request a Certificate of Attendance and session summary report.

Deadline
The deadline for claiming AMA PRA Category 1 CreditsTM and EHA-CME Credit Points is Friday, April 11, 2014.
